Photo / Video Policy: First Parish is a vibrant community that uses photos, videos and recordings during church-related celebrations and sometimes for publicity, brochures and on our website. Unless I communicate otherwise in writing to the CRE, I understand that the taking and usage of pictures/video of my child (or children in my care) is permitted, but that the church will never identify my child(ren) by name.

Medical Emergency and Treatment: If the emergency contact or I cannot be reached in time, I hereby authorize the administration of emergency medical aid to my child. In emergencies requiring immediate medical attention, my child will be taken to the nearest hospital emergency room. I agree to accept financial responsibility for any and all emergency medical care so provided. I also authorize the FPB trip supervisors to administer medications to my child, if any, listed below on this form.
